New Hampshire Home Care Providers, LLC is seeking a Coordinator of Client Services to oversee the daily coordination and supervision of home care services for our clients throughout New Hampshire.
This position plays a central role in ensuring that clients receive dependable, compassionate care while supporting and guiding our team of dedicated caregivers. The Coordinator works closely with clients, families, and field staff to coordinate services, manage scheduling, and maintain the high standards of care our organization is known for.

The Homemaker is a paraprofessional team member who provides household and supportive services to clients under the supervision of the Coordinator of Client Services.
Homemaking services focus on maintaining a healthy living environment and supporting the client’s well-being through assistance with household tasks, meal preparation, and observation of changes in the client’s condition.
Homemakers are assigned to clients by the Coordinator and follow a written plan of service that outlines specific duties and responsibilities for each client.

Homemakers are responsible for performing services according to the written assignment and agency guidelines, including:
Homemakers also confirm scheduled visits with the Coordinator on a weekly basis to ensure proper coordination of services with other care personnel.
Staff must notify the agency promptly if illness, emergencies, vacation, or other circumstances will affect their ability to provide scheduled services.
Additional duties may be assigned as necessary.
