Coordinator of Client Services

 New Hampshire Home Care Providers, LLC is seeking a Coordinator of Client Services to oversee the daily coordination and supervision of home care services for our clients throughout New Hampshire.

This position plays a central role in ensuring that clients receive dependable, compassionate care while supporting and guiding our team of dedicated caregivers. The Coordinator works closely with clients, families, and field staff to coordinate services, manage scheduling, and maintain the high standards of care our organization is known for.

Position Overview

The Coordinator of Client Services is responsible for the day-to-day management of client services, including scheduling caregivers, processing referrals, supervising service delivery, and assisting with staffing and quality improvement initiatives.

This role ensures that services are coordinated efficiently while maintaining the highest standards of professionalism and client satisfaction.

Reports To:  Office Manager and Administrator
Occupational Exposure Category: C – No expected exposure to bloodborne pathogens

  

Key Responsibilities

  • Receive and manage new  client referrals.
  • Assign appropriate field  staff based on client needs and caregiver availability.
  • Communicate with clients, family members, and referral sources to coordinate services.
  • Supervise client  services in accordance with agency policies and quality standards.
  • Maintain accurate  documentation of communications with clients, families, and staff.
  • Manage and maintain caregiver scheduling to ensure service coverage.
  • Monitor staffing levels  and notify the Administrator when additional hires are needed.
  • Assist with interviewing  and hiring service personnel.
  • Orient new staff to  agency policies and procedures.
  • Supervise staff to  ensure high-quality performance standards are met.
  • Maintain professional communication and strong working relationships with all staff.
  • Remain accessible by telephone during office hours to support service needs.
  • Participate in the  agency’s on-call rotation on an equitable basis.
  • Carry a communication device when away from the office and respond within one hour when contacted.
  • Develop relationships with community organizations and referral sources.
  • Perform additional duties as assigned by the Administrator.

  

Qualifications

  • High school diploma or equivalent required
  • Minimum two (2) years of experience supervising personnel or providing personal care services
  • Strong organizational, communication, and problem-solving skills
  • Ability to coordinate multiple schedules and service priorities
  • Successful completion of a Criminal Background Check
  • Successful completion of  a BEAS Registry Check

  

Functional Abilities

  • Ability to hear and communicate effectively in English
  • Ability to read printed materials in 12-point type or larger
  • Ability to travel to prospective clients’ residences when necessary

Personal Care Worker

Position Overview

The Homemaker is a paraprofessional team member who provides household and supportive services to clients under the supervision of the Coordinator of Client Services.

Homemaking services focus on maintaining a healthy living environment and supporting the client’s well-being through assistance with household tasks, meal preparation, and observation of changes in the client’s condition.

Homemakers are assigned to clients by the Coordinator and follow a written plan of service that outlines specific duties and responsibilities for each client.

Reports To: Coordinator of Client Services
Occupational Exposure Category: B – Limited exposure to bloodborne pathogens

Key Responsibilities

Homemakers provide a variety of household and supportive services, which may include:

  • Grocery shopping with attention to budgeting and nutrition
     
  • Preparing nutritious and appetizing meals
     
  • Assisting with reminders related to diet restrictions, medication schedules, and recommended exercises
     
  • Observing and reporting changes in a client’s condition, mood, or behavior
     
  • Providing encouragement and emotional support during periods of loneliness, illness, or bereavement
     
  • Performing routine housekeeping tasks necessary for a clean and safe home environment
     
  • Laundry services including washing, drying, folding, and putting away clothing
     

Service Documentation and Communication

Homemakers are responsible for performing services according to the written assignment and agency guidelines, including:

  • Following the established plan of service for each client
     
  • Recording tasks performed and observations during each visit
     
  • Signing and completing visit documentation accurately
     
  • Communicating regularly with the Coordinator regarding client needs or concerns
     
  • Reporting emergencies immediately to the Coordinator or agency office
     
  • Maintaining accurate time and mileage records
     
  • Following agency policies, procedures, and client-specific instructions
     
  • Working within the established organizational structure and communication channels
     
  • Wearing appropriate clothing and an agency identification badge while on duty
     

Homemakers also confirm scheduled visits with the Coordinator on a weekly basis to ensure proper coordination of services with other care personnel.

Staff must notify the agency promptly if illness, emergencies, vacation, or other circumstances will affect their ability to provide scheduled services.

Additional duties may be assigned as necessary.

Qualifications

  • Prefer a high school diploma or equivalent
     
  • Must be at least 18 years of age
     
  • Prefer one (1) year of experience in a work environment
     
  • Must complete required agency training
     

Functional Abilities

  • Ability to read printed materials in 12-point type or larger
     
  • Ability to hear and communicate clearly with clients and staff
     
  • Ability to stoop and bend when performing household tasks
     
  • Ability to travel to clients’ residences
     
  • Ability to carry items weighing up to 10 pounds, including carrying items upstairs when necessary
